Health History

We've recently switched over to a new system called FrontDesk.

When you click HERE, you will either login to an exisiting profile (if you have climbed with us in the past, please try logging in with the email address you had when you reigstered last time), or create a new profile.

Once you are in your profile, you can complete our Health History and Emergency Contact form. We've also switched over to digital waivers to help with storage and reducing the use of paper. We'll either send you the link to the online waiver, or you can sign in in your profile.

Please let us know if you experience any issues when using our new system. We're hoping to make it as efficient and easy as possible.

Our health history form is required to be completed by all participants of our programs. If you can't login, you can download the form, read it completely and hand the completed original to your guide when you meet them.
