Below are required forms and supporting documentation for some of our programs - dependent on where you go.

Required Paperwork For Those Participants Climbing in Minnewaska State Park

All participants climbing with us at Minnewaska State Park will need to download and complete the Minnewaska Waiver.

If you are in a group of 8 or more climbers, please download and print out one copy of the group sign in form. Please have each climber print and sign their name on this list. It will help expedite things in the morning

Minnewaska Release

This one is required for all participants who will be climbing at Peterskill, in Minnewaska State Park. If you are under age 18, it must also be signed by your parent or legal guardian in the appropriate location.

Stone Mountain Farm Information

For our group rock climbing programs, one of the area we operate a is Stone Mountain Farm. They are a private farm in the Shawangunks offering cliffs for rock climbing and camping as well. If we are scheduled to climb with you there, please review the following information

To make the day more efficient and maximize your time climbing, please login to our new system FRONTDESK.

When you click HERE, you will either login to an existing profile (if you have climbed with us in the past, please try logging in with the email ad dress you had when you registered last time), or create a new profile.

Once you are in your profile, you can complete our Health History form and digital Participation Agreement.

  • Please let us know if you experience any issues when using our new system. We're hoping to make it as efficient and easy as possible.
  • If that's not possible, you can download the following forms required for your day and hand the originals to your guide when you meet them.

This one is requested for all participants. Please be sure to write legibly, we need to be able to read all the information.
